- A statement that you believe in good faith the use is not authorised.
- A statement, under penalty of perjury, that the information is accurate and that you are the owner or authorised to act for the owner.
- Your physical or electronic signature.

Counter-notice
If material you provided was removed and you believe it was a mistake, you can send a counter-notice with the same details and a statement that you consent to the jurisdiction of the appropriate court.
Repeat infringement
We may close accounts of customers who repeatedly infringe others' rights through the service.
